Simple ways to protect yourself

How to influence the subconscious

The hypnotic effect occurs through the invocation of subconscious trust and the establishment of so-called rapport. The theory, techniques and practice of hypnosis are inextricably linked with this concept. Rapport is the connection established by the hypnotist with the hypnotized person.

With the establishment of rapport, the hypnotized person becomes more receptive to the suggested words of the hypnotist, readiness to carry out suggested tasks, and loses reaction to other external stimuli. To create unconscious trust and rapport, the hypnotist identifies himself with the hypnotized person - makes his behavior similar, adjusts his posture, gestures, movements, and breathing patterns.

Adjustment to posture and movements

The simplest technique in creating subconscious trust is to adjust to the pose of the person being hypnotized - it must be copied, directly or crosswise (that is, like in a mirror). If, for example, the client's left leg is on top when crossing his legs, when cross-adjusting, the hypnotist should have his left leg on top. But a person rarely sits still, so movements also become the subject of adjustment. This adjustment is more difficult; you need to be very careful, watching and repeating gestures and facial expressions, and at the same time not letting your partner realize it.

The easiest way is to adapt to a person’s gestures, and when copying a gesture, you don’t have to complete it, but only outline and repeat the direction. It is convenient to reproduce the frequency of blinking - the opposite party is almost never aware of this.

When performing training exercises, you need to work on both types of adjustments. Watching the person, begin to breathe evenly in time with his inhalations and exhalations. Then select a subject with an increased breathing rate and try to copy the rhythm of inhalation and exhalation by shaking your leg or moving your finger.

Voice and speech

The voice is the hypnotist's working tool. A clear, strong, pleasant voice, easily moving from tone to tone, introduces the hypnotized person into a state of trance. Speech should be clear, unhurried, and convincing. It is necessary to develop your own voice, which will act hypnotically, train it in accordance with these requirements, learn to change intonation, make it rhythmic if necessary.

There are two more rules used in hypnosis:

  1. The speech should not contain negatives. Negations do not exist in the subconscious; it does not perceive them. An exercise for training such speech is to avoid using the particle “not” at all for some time, and instead of using negative words, use others that are suitable in meaning. Then return to normal speech, but when talking about experiences, do not use the negative particle.
  2. Phrases should be general in nature, not specific to the situation. The person being hypnotized must have unoccupied space for his impressions and feelings. This facilitates a faster introduction into trance.

At the same time, look at the bridge of the person’s nose in the portrait for one minute, then abruptly move to another portrait and fix your gaze on the bridge of the person’s nose in the other portrait. And so repeat the exercise with portraits of other people, while you need to move your gaze sharply, with lightning speed.

— On what day does coronavirus pneumonia usually develop, requiring serious medical care? I often hear that lung damage begins even before the first symptoms of the disease.

— Pneumonia of various etiologies, including Covid, can develop at any time within three weeks from the moment of infection. But in cases where we are talking about its severe course, most often it is the fifth to eighth day. The most serious complication of pneumonia associated with COVID-19, acute respiratory distress syndrome (ARDS), develops within seven days of the onset of the disease. The time interval of a week from the moment of exposure to the factor to the development of the clinical picture is one of the clear diagnostic markers of ARDS.

The severity and prevalence of the inflammatory process in the lungs directly depend on the so-called viral load, that is, the number of viral particles that have entered the human body. The state of the immune system, genetic characteristics, and the presence of concomitant diseases also play a role. Accordingly, the more viral particles, the weaker the immune system, the faster the process develops and the more difficult it is. Chronic diseases or some genetic characteristics, congenital pathologies can also lead to a more severe course of COVID-19.

Indeed, lung damage begins even before the first symptoms of the disease appear, but this is quite natural. There will be no clinical symptoms if there is no lesion. I would formulate this idea differently: the main feature of COVID-19 is that the patient’s clinical picture often does not correspond to the degree of lung damage. This phenomenon is manifested, for example, by unexpected findings of bilateral pneumonia during random X-rays or computed tomography of the lungs. That is, the person feels well, there are no serious complaints, but he went to a health care facility with some other disease, they performed a CT scan or x-ray and found pneumonia. There were no characteristic signs of inflammation (cough, fever, shortness of breath). This feature of coronavirus infection puts it in a unique position when it is necessary to take a set of steps for timely detection.

If the saturation is 95-100%, then the blood is sufficiently saturated with oxygen. An indicator of 94% and below indicates that oxygen from the alveoli does not pass into the blood and lung function is impaired. A person has a CT scan or x-ray to evaluate the condition of their lungs.

By the way, many smartphones, fitness trackers, and smart watches have a pulse oximeter function. For example, some smartphone models have a heart rate sensor on the back panel next to the camera. You need to put your finger on it and use the installed application to measure the saturation level and heart rate.

In fact, the capabilities of gadgets are limited, however, they allow you to get your bearings. We compared the results on one of the smartphones with those shown by the resuscitation equipment - the difference is plus or minus 1-2%. We came to the conclusion that we should not completely rely on the readings of gadgets, because measuring saturation is critically important for life, but their capabilities can be used. If blood oxygen saturation has decreased significantly, in addition to fever and cough, there is every reason to consult a doctor.

— In what cases is a decision made to connect a patient with COVID-19 to a ventilator? Are we talking about borderline states?

— It all depends on how low the saturation is. If the indicator drops below 95%, at least a turn on the stomach, in the so-called prone position, and oxygen therapy are required. Less than 85% - we immediately take them from the emergency room to the intensive care unit, the person most often ends up on a ventilator. The decision is made by an anesthesiologist-resuscitator. Thanks to the transfer to hardware support, many patients manage to survive a life-threatening condition. If you notice negative changes in the lungs in advance, you can get by with oxygen therapy and lying on your stomach.

— Recent American studies show a high mortality rate among patients with COVID-19 who were treated with artificial ventilation. In your opinion, is mechanical ventilation the last chance for salvation?

— In principle, it is incorrect to compare mortality among those patients who were on a mechanical ventilation device and those who did without mechanical ventilation. These are two completely different groups. People who, for some reason, cannot breathe on their own need mechanical breathing; their gas exchange in the lungs is critically impaired: oxygen cannot pass from the pulmonary alveoli into the blood, and carbon dioxide, on the contrary, from the blood into the alveoli. This is a life-threatening situation, so transfer to mechanical ventilation is really, to some extent, the last chance for salvation.

You need to understand that patients with pneumonia on mechanical ventilation are a priori severe patients. The development of acute respiratory distress syndrome as the most dangerous complication of COVID-19 significantly aggravates the course of the disease and increases the risk of an unfavorable outcome. However, ARDS is not actually a specific syndrome for COVID-associated pneumonia. It can be caused by a number of surgical and therapeutic pathologies, including pneumonia of other origins. By itself, transferring to mechanical ventilation does not cure either ARDS or pneumonia, but it allows a critically ill patient to simulate the breathing process, and at this time treat the underlying pathology, thereby giving time to organs and systems to recover from a state of shock.

As for SARS-COV-2, which causes COVID-19, there are currently no drugs with a good evidence base against this virus. We rely on the response of the person's own immune system. Hardware support (essentially artificial life support) gives the body time to cope with the viral load.

— Are there methods to delay the transfer of patients with coronavirus to artificial ventilation?

— Ventilation is an extreme degree of respiratory support. Fortunately, among patients with COVID-associated pneumonia, there is not a very large percentage of those requiring mechanical ventilation. Timely delivery of a humidified air mixture with increased oxygen content through nasal catheters or using a face mask allows a very large number of patients to cope with this condition and recover simply on oxygen therapy.

Another method that we actively use in intensive care units is the prone position, when patients are placed on their stomachs. The ventilation-perfusion relationships in the lungs change, as a result, those parts of them that were poorly supplied with oxygen in the supine position are better ventilated. This improves the transfer of oxygen from the alveoli to the blood, that is, oxygenation. These two simple methods can treat a fairly large percentage of people with severe pneumonia.

Only those patients in whom oxygen therapy using nasal catheters or a face mask and turning onto their stomach were ineffective are transferred to mechanical ventilation. If these measures do not allow us to improve oxygenation, we decide to switch to machine respiration, which allows us to simulate the respiratory function and increase the percentage of oxygen in the supplied mixture.

By the way, with the help of nasal catheters we can increase the oxygen content in the inhaled air up to 40%, with the help of a face mask a little more - up to 50-60%. The ventilator allows you to set any oxygen content. If a person has very severe lung damage, it could be 70%, 80%, or even more. A person will receive as much oxygen as he needs.

“Perhaps only a highly qualified specialist clearly knows when a person needs to be transferred to mechanical ventilation. After all, delay, like haste, can not work in a person’s favor.

— Indeed, these should be highly qualified anesthesiologists and resuscitators with experience. In fact, there are a number of even more subtle and informative indicators, besides saturation. For example, in intensive care units we take arterial blood for laboratory tests, its acid-base state and gas composition are analyzed. If the partial oxygen tension is less than a certain level, this is an absolute basis for transferring to artificial ventilation.

Arterial blood gas analysis is a practical routine skill of an anesthesiologist-resuscitator. Otherwise, the specialist would not be able to confirm his qualifications upon completion of the internship.

— Is training of doctors carried out in the regions? New knowledge on the management of coronavirus patients is emerging every day, and it is important to convey this experience to colleagues.

— Each region is assigned consultants who provide methodological and practical assistance and, if necessary, can go to a specific hospital. For example, I am assigned to the Gomel region. In addition, we record video lectures for doctors. There is a lot of work, but it is coordinated, the doctors know what to do.

Regarding transfer to mechanical ventilation, after 2009, when there was an outbreak of pneumonia caused by swine flu, our service gained unique experience. Over the years we have come very far. Belarus has accumulated knowledge and techniques for caring for patients with severe respiratory distress syndrome, so we were well prepared for this pandemic. We have enough artificial lung ventilation devices and qualified personnel.

Another thing is that we are faced with some features of respiratory distress syndrome in COVID-associated pneumonia. In 70-80% of patients, we do not see the same pattern of lung damage that was seen with influenza ARDS. Impaired oxygenation is largely associated not with lung damage itself, but with impaired perfusion—blood flow through the pulmonary vessels, which is why patients with COVID-19 are prone to thrombosis.

According to our foreign colleagues, and we see this too, the frequency of thrombosis in patients with COVID-19 who were in intensive care units is about 30%. That is, every third patient with severe COVID-19 has some kind of thrombotic complications. These may be deep vein thrombosis, pulmonary embolism, acute coronary syndrome, heart attacks or ischemic strokes. Another feature of coronavirus infection is that with COVID-19 there is multiple organ damage. That is, not only the lungs suffer, but also the heart, kidneys, and nervous system. In this case, mechanical ventilation will not help; the rheological properties of the blood need to be improved.

We approached the treatment of such patients from a different angle - we strictly control blood coagulation parameters and the severity of inflammation. Today, all patients with covid pneumonia are required to receive blood thinning medications. In general, this has a good effect on the course of the disease; in such patients there are fewer cases of deterioration of the condition.

— How many patients can be disconnected from mechanical ventilation and transferred to a ward?

— Contrary to popular belief among ordinary people, mechanical ventilation is not a death sentence; quite a lot of patients can be disconnected from the device. However, you need to understand that the weaning process can take up to two-thirds of the total time spent on mechanical ventilation. Removing a patient from a ventilator is not easy, it is an art.

A person who is put on mechanical breathing in a critical condition develops muscle atrophy within a few days. This is especially true for older people, who are already characterized by age-related natural loss of mass and muscle strength. If a patient has been on a ventilator for a week, it becomes very difficult to get his muscles working again. The process of weaning the elderly from artificial ventilation takes days and weeks. A rehabilitator always comes, we do gymnastics, etc.

— I read that a person on a ventilator can control his condition, and in those cases when he feels that he can breathe on his own, ask him to be disconnected from the device for a while. Is it possible?

— This way they are disconnected from the device after short-term ventilation, for example, after operations, but this is unlikely in relation to respiratory distress syndrome. Absolutely all patients with pneumonia, when transferred to artificial ventilation, are in medicated sleep (this is not a coma, but deep sleep). The fact is that tracheal intubation is a painful and unpleasant procedure. For severe pneumonia, deep medicated sleep lasts at least 48 hours so that the body can adapt. A person needs complete rest, this allows his organs and tissues to begin to recover.

Doctors monitor the patient's condition. We had no precedent for a person to wake up, snap his finger, and say that he would breathe on his own, as in the film. The doctor’s task is to teach him to breathe again. This is a well-coordinated team effort - mine as a resuscitator, a rehabilitation specialist, and a nurse.

— How is a patient weaned from mechanical ventilation? Are there any breathing training techniques?

“We now have very good respiratory equipment; the devices can either completely breathe for the patient or support his own breathing. A person can be disconnected from mechanical ventilation after the signs of the disease that caused the need for hardware support begin to subside. For example, if a patient with severe pneumonia has a decrease in temperature and blood tests have improved, we have reason to say that he is on the mend. We reduce the dose of medications that caused medicinal sleep and begin to wake him up - very carefully so that the person does not get scared.

As the person wakes up, the device records attempts to breathe. A ventilation mode is established that supports a person’s independent breathing, gradually teaching him to breathe again. The patient will remain on a ventilator for several more days. All this time we will talk to him, wait for the moment when he can breathe on his own. Before the pandemic, relatives were allowed to come. This had a very beneficial effect: seeing a familiar face, the person was encouraged, and he had a desire to get better quickly. Now, of course, it’s more difficult for patients. Waking up in intensive care is already stressful for anyone. Now, when a person comes to his senses, he sees medical personnel around him in protective suits, respirators, and shields.

The process of weaning from the device is gradual. As I noted, it can take from several days to several weeks, depending on the age of the patient and the severity of the disease. First, we train breathing using the apparatus, gradually changing the parameters. Relatively speaking, a healthy person takes 16 breaths per minute. We set a special ventilation mode so that the device breathes for the patient 12 times, and he takes the remaining four breaths himself. Then we begin to reduce hardware support and eventually set the spontaneous breathing mode. And only when a person gains strength, the question is raised about completely removing him from mechanical ventilation.

— Do patients with COVID-19 need long-term rehabilitation after this?

— All patients who go through critical conditions in intensive care, including after pneumonia, need long-term rehabilitation. And the lungs need to be restored and recovered from stress. Every year we have patients with severe pneumonia and distress syndromes; their rehabilitation system is well established.

— Could extracorporeal membrane oxygenation (ECMO) become an alternative to mechanical ventilation in the treatment of coronavirus patients?

— ECMO is widely used, in particular in cardiac surgery. As for severe forms of ARDS, extracorporeal membrane oxygenation is indicated for patients with severe forms, when oxygen does not pass from the alveoli into the blood, that is, when maintaining gas exchange with mechanical ventilation is ineffective. However, this procedure is not simple, very expensive and requires specially trained personnel. In no country in the world, ECMO is not a panacea and cannot serve as a full-fledged alternative to mechanical ventilation, because the severity of the condition is associated not only with respiratory disorders, but also with thrombosis, thromboembolism and disseminated intravascular coagulation.

The method can be used specifically in individual cases under a number of conditions. For COVID-19, ECMO is currently used infrequently. Since the outbreak of the epidemic, more than 800 such procedures have been performed on confirmed patients worldwide, including over 200 in Europe. Considering the number of cases, this is a very small percentage.

— According to experts, most of the globe will be infected with coronavirus. Fortunately, 80-85% will experience the disease asymptomatically or mildly. What recommendations can be given to the population to prevent a severe course of the disease and, accordingly, not to end up on mechanical ventilation?

— The severity and prevalence of pneumonia and outcome depend on the viral load, immunity and the presence of chronic diseases. The first task is not to fall into a category with potentially severe pneumonia. Each person should analyze whether he or his family members have risk factors (age, presence of chronic diseases). More often, patients with diabetes mellitus, pathology of the kidneys, cardiovascular system, immunodeficiencies, older people, and people with obesity are seriously ill.

It will be difficult to avoid the virus. Now many experts actually predict that about 70% of the world's population will get sick. This virus tends to fit into the usual seasonal incidence, so if we don’t see it today or tomorrow, we’ll see it in a year. In any case, we need to try to reduce the viral load using the recommendations given by our healthcare system. These are very simple rules that you can follow and still work and live an active life.

For example, it is advisable for risk groups to wear masks in public places. For healthy people who are not at risk, this is not necessary. At the same time, I urge you to be responsible: if you feel unwell, show respect for society and also wear a mask so as not to infect those around you.

If you are not indifferent and care about the environment, use reusable masks in your everyday life. Now there are plenty of them in stores, for every taste and color, and they are inexpensive. The history of hypnosis goes back at least three millennia. It is known that the ancient shamans and priests of Ancient Egypt were perfectly able to put people into a trance; it was successfully used in Ancient India and Tibet. Hypnotic sessions were conducted by healers of Ancient Rome and Ancient Greece. Then, with the help of hypnosis, they raised the morale of soldiers, looked into the future and treated diseases.

The term “hypnosis” itself appeared only in 1843 after a year earlier the English doctor J.   2. The individual needs to concentrate on breathing. To quickly enter a trance, special breathing exercises are used. Breathing should be uniform and deep. As you exhale, the man or woman becomes completely focused, and as you inhale, you relax again.
  3. The person imagines that his mind consists of several parts. The highest level is the active part (consciousness), and deep hypnotic sleep is at the lowest level.
  4. The person being hypnotized needs to move from one level to the second. You need to relax each time and then explore the level. You need to feel what is inherent in your personality. Knowing the level is a state of trance.

Time will pass unnoticed if the entrance to the border state has been completed correctly. To get out of this state, a person concentrates on inhaling. As the lungs fill with air, the personality carefully rises through the levels. When the turn reaches consciousness, the eyes open.

Eye rotation and breathing

Another practice for use at home. A man or woman should be calm. Starting position: lying or sitting. The person's head must have support, and during a trance it is better not to move. This technique is used for self-hypnosis: a man or woman closes his eyes and relaxes all the muscles of the body. The greatest relaxation occurs when you exhale.

After preparation, the eyes open - you need to sharply look up and hold your breath. After 5-7 seconds. the eyes slowly lower and relax. The eyelids close. The person inhales, then exhales and stays in this position for 5 seconds. As you inhale, your eyelids lift again. The gaze is directed to the ceiling and the breath is held again for 5-6 seconds.

The eyes close - the person concentrates all his attention on breathing. The body slowly relaxes. It is necessary to listen to the body, feel every movement, tingling, trembling. With relaxation comes freedom from unnecessary thoughts: trance eliminates empty, distracting thoughts.

Relaxation lasts for at least 15–20 minutes, after which, to get out of this state, you need to concentrate on exhaling. Gradually the person returns to a conscious state.

Multi-stage technique

The 4-step technique is more complex. It is used to independently enter a borderline state. Stages of going into trance:

  1. The person takes a comfortable position (lying down), it is better to lie on the floor or any hard surface. In this position, you need to monitor your breathing. It should be smooth, slow. While inhaling, the following sounds are pronounced: “So-o-o-o,” and when exhaling, “Hum-mm.” The sounds are repeated until breathing becomes natural and does not have to be monitored. At this stage, unnecessary thoughts and worries should disappear.
  2. The person needs to completely get rid of movements in the second stage. With the effort of the mind, you need to stop each one so that the body plunges into absolute peace. Relaxation begins with the facial muscles and gradually reaches the fingertips. When the muscles and muscles relax, say the words to yourself: “My body is relaxing, my eyelids are getting heavier.” Thoughts of relaxation should be repeated until the body becomes sluggish, almost immobile. The entire relaxation cycle is repeated at least 4 times.
  3. As a person, after the body is completely relaxed and the mind stops generating new distracting thoughts, you need to say the words of the mantra: “Om.” It is repeated 14 times. When the syllable interrupts all thoughts, the person moves on to the final stage of trance.
  4. The mantra is repeated at the last stage. New images should appear in your head - the rays of the sun or the solar circle. This visualization is not easy for everyone the first time, but over time the image will become clearer and clearer.

In a state of absolute trance, you can imagine any image, desire, thing - all this will take root in the subconscious. One way or another, a person will be able to get what he wants soon. To break out of trance, you need to focus on your own breathing.
